Cognitive Behavior Therapy (CBT) has been shown to be an effective treatment approach for a variety of disorders.

phone surveyThe purpose of this tool is to maximize several components of CBT therapy, although it can also be used by other providers in the behavioral health and medical fields.

It is to be used by professionals as an adjunct to treatment, not as a stand alone program. With the forms provided clients can record this information on their mobile device whenever necessary.

They can record triggers, identify and choose a strategy, evaluate the success of the chosen strategy and then transmit this information to their therapist for use in their next session along with additional concerns that they wish to discuss.

The mobile device itself can become a cue for improved symptom/behavior management and coping. Most forms offered here were developed with CBT in mind but can be used in any therapeutic context.

Several additional forms are available: mood lability, pain management, and general treatment response (which can include medication).

Since caregiver report of children's behavior can be influenced by the days or hours before their appointment, forms are included to assist caregivers with two common behavioral issues: tantrums and bedtime. Depending on user feedback, more forms will be added in a future update of this program.

Homework and keeping “data”

Homework is a central component of CBT. Doing “homework” has been an ongoing struggle with therapy and this tool can help improve client compliance/follow-though. Clients often say they forgot, they did not have pen and paper handy, they weren’t in a context that was conducive to data collecting, etc.

Additionally, as the time interval between sessions increases, client report becomes less reliable and more influenced by events immediately preceding an appointment. These tools capitalize on the fact that people are rarely without their mobile devices. They often have them by their side when they sleep, at restaurants, in the rest-room, and during their work and travel.

Data can be entered with a few clicks instead of hunting down forms and a pen. Text entry in many cases has replaced the telephone, letter, email and IM as a primary means of communication – particularly with younger users.

Treatment as a collaboration

While there are numerous self help resources and workbooks available for therapists and the public to use (either in session or out), this website provides a way that therapists and their clients can work more collaboratively.

This program allows a client to use their mobile device as a ‘touchstone’ to provide an ‘in time’ stream of invaluable information that can then be reviewed in treatment sessions. This website is an effort to combine the above factors to increase your success with your clients.

In essence this is the way the service works:

You as a provider become a member of LIFESUDS. There is a minimal recurring membership fee of $4.99 monthly for less than 20 clients or $9.99 for an unlimited number of clients.

You access the program from your desktop or laptop computer.

There are a number of pre- designed forms for different problem areas including: Anger Tracker, Anxiety Tracker, Bullying Log, Child Bedtime Log, Exposure Record, Mood Change Tracker, Pain Management, Panic Log, Urge Log, Tantrum Tracker and Treatment Response Log. There is also a customizable form for your use.

You invite your clients/patients to become a member (at no cost to them). You and your client decide which forms and what data would be helpful to collect in between sessions. Your client then downloads the appropriate form (i.e. anxiety log) to their mobile device. If they do not have a smart phone, they are able to complete the form on their computer.

Your client enters data “in vitro” – at the time of or just after an incidence of the target behavior. They enter the data on their device and store/transmit it to you over our secure (fully encrypted) server.

You receive the (encrypted) data in easily readable and summarized form (if more than one incident is entered) for review in your next session. 

This not only provides you with ongoing information on how your client/patient is doing in between sessions, but also can help you identify issues to be addressed in your next appointment.

Since there are a variety of platforms available we have developed a way you can make this tool available to your patients regardless of the type of mobile device they have, as long as they have connection to a cellular network. If they are momentarily out of range of a network, the data can be saved on the device for later transmission. And as indicated above, a computer can be used in the absence of a mobile device for data recording and submission.

For certain situations i.e. recording success getting child to bed, planned exposure therapy the client can set up their device to remind them to enter the data.

Additional features

In addition to the CBT forms, several other forms are provided for your use. We have supplied a medication/treatment log for patients to let you know on a regular basis how they are doing with medication. This can be especially helpful if they are trying a new medication or treatment.

There is a mood lability tracker to help you evaluate serious mood disorders such as bipolar disorder. This form can be used by the patient themselves or by someone else, such as a parent, to track mood swings.

Again the ability to have ongoing data, at the time interval you decide, allows for more accurate reporting. A couple of forms are included for use by caregivers for two common issues (bedtime, tantrums). If these are found to be helpful, more of these can be added in future updates of this program.


In using these tools, it is important that you follow the usual HIPAA requirements for client confidentiality. This program includes 256 bit SSL encryption.
